Day Training Policies

Day Training Waiver

Ready Set...Train! LLC will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Ready Set Train! is insured for any unintentional or negligent errors, omissions, or incorrect assertions Ready Set…Train! LLC will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by Ready Set Train! and understand the inherent risks of owning an aggressive/reactive/fearful dog, including but not limited to the risk of dog bites to myself, other people, or other animals and consequently I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify and hold harmless Ready Set…Train! LLC of any and all claims of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

Behavior History/Vaccination Policy

I understand that it is my responsibility to provide Ready Set…Train! with information about my dog that is one-hundred percent accurate and true in the intake form that is incorporated into this agreement by reference, and I will update Ready Set...Train! LLC with any new information regarding my dog's behavior. I will provide proof of my dog’s rabies and DHLPP vaccination and a negative fecal test before my first visit. Cancellation Policy

I understand that I must give at least 48 hours notice of any cancellations or scheduling changes. If a notice of 48 hours is not given, I understand that Ready Set Train! has the right to charge for that training session or a package credit will be applied to that training and no refund will be given. Exceptions can be made due to emergency and sickness, but this will be determined by the trainer at the time of the cancellation request.

Transportation

I authorize Ready, Set...Train! and its employees, contractors, and representatives to transport my dog(s) to and from training sessions, classes, day training or other agreed-upon destinations as part of the services I have requested.

I understand that transportation may occur in personally owned vehicles operated by Ready, Set...Train! Staff.

I understand that Ready, Set...Train! will make reasonable efforts to ensure my dog's safety during transportation, including the use of appropriate restraints, crates, seatbelt systems, barriers, leashes, and other safety equipment as deemed appropriate by staff.

I understand that animals can behave unpredictably and that transportation carries inherent risks, including but not limited to:

Vehicle accidents
Escape attempts
Injury during loading or unloading
Illness, stress, or anxiety associated with travel
Injury caused by interactions with other animals

Emergency Vet Care

In the event of illness, injury, or emergency involving my dog while in the care of Ready, Set...Train!, I authorize Ready, Set...Train! to seek veterinary treatment as deemed necessary if I cannot be reached promptly.

I agree to assume full financial responsibility for any veterinary expenses incurred on behalf of my dog. I am willing to approve up to: Dollar Amount ________ in vet care for my dog.

I understand this contract is the entire agreement between the two parties and there is no other valid agreement. Any provision that is struck out of this contract the remainder of this contract still stands. Any scanned or electronic copies of this contract are legitimate and will function as the original. By signing below, I am agreeing to all of the above statements, and I state my intent to enter into a training contract with Ready Set Train! LLC. Please digitally sign your name and date on the lines below. This contract is validated by the signatures below in total and as approval for future services without additional written authorization.
